eClaims users: Your version of DENTRIX does not support the use of an NPI number. However, completing the form below will allow us to include your NPI number and continue processing your electronic claims.
If you have multiple NPI numbers you can enter them with the form below. After submitting the first NPI number you will have the option to enter additional NPI numbers.
